If you were born in the Town of Grafton, or your parents resided in Grafton at the time of your birth, you may obtain a birth certificate from the Grafton Town Clerk’s Office. The fee is $5.00. If you call ahead the certificate will be ready when you arrive.
You may request the certificate by mail by completing the Birth Certificate Request Form and return it with payment and self addressed stamped envelope to the Town Clerk’s Office. All mail requests are processed the same day we receive them.
