If the individual died in Grafton, or was a resident of Grafton at the time of death, you will be able to obtain a death certificate from the Town Clerk’s Office. The fee is $5.00. If you call ahead the certificate will be ready when you arrive.
You may request the certificate by mail by completing the Death Certificate Request Form and return it with payment and self addressed stamped envelope to the Town Clerk’s Office. All mail requests are processed the same day we receive them.
